User guide for the Bupa HealthCare Card (Hospitalisation)

Please call the Bupa Hospitalisation Pre-admission hotline：2517 5328
Fill in and return the Hospitalisation Pre-admission Form
Receive a Guarantee of Payment (GOP) from Bupa after about one working day
Present the GOP and your valid Bupa HealthCare Card for registration upon admission to the hospital
Sign the claim form completed by the hospital upon discharge
