User guide for the Bupa CrystalNet Card* (Hospitalisation)

• Present your valid Bupa CrystalNet Card to the network doctor upon registration
• If you need to be hospitalised for treatment, the network doctor will complete the Pre-authorisation Form for the member and send it to Bupa for approval
• The network doctor will contact the member after receiving the approved Pre-authorisation Form from Bupa about one working day later
• Member should present the Pre-authorisation Form and the valid Bupa CrystalNet Card for registration upon admission to the hospital
• Sign the claim form completed by the hospital upon discharge

* only applicable to four CrystalNet hospitals and pre-approved treatments. If you need to be hospitalised in hospitals outside the network, please refer to the claim procedure.

• Present your valid Bupa CrystalNet Card to the network doctor upon registration
• If you are receiving any treatments referred by the network doctor where relevant specialty is not listed in the latest "List of CrystalNet Service Providers", you are required to get pre-authorisation from Bupa
