User guide for the Bupa HealthNet Card * (Hospitalisation)

• Present your valid Bupa HealthNet Card to the network doctor upon registration

• If you need to be hospitalised for treatment, the network doctor will complete the Pre-authorisation Form for the member and send it to Bupa for approval
• The network doctor will contact the member after receiving the approved Pre-authorisation Form from Bupa about one working day later
• Member should present the Pre-authorisation Form and the valid Bupa HealthNet Card for registration upon admission to the hospital
• Sign the Claims form completed by the hospital upon discharge

* only applicable to five network hospitals and pre-approved treatments. If you need to be hospitalised in hospitals outside the network, please refer to the claim procedure.

User guide for the Bupa HealthNet Card (Clinical benefit)

• Present your valid Bupa HealthNet Card to the network doctor upon registration
• If the cost of diagnostic imaging and laboratory tests is over HK$1,000, or you are receiving any treatments by the network doctor where relevant specialty is not listed in the latest "List of HealthNet Service Providers", you are required to get pre-authorisation from Bupa
• If the cost of operations done in clinics or day case units is over HK$2,000, you are required to get pre-authorisation from Bupa
